I assume that the childs bike in referred to has 20" wheels, so the questions are - is there enough room on the rear wheel to fit a multi-freewheel, and what width is the chainring?
To convert to a geared system you will need a short cage rear derailleur with bracket, new chain of 3/32 width, a gear shifter and a multi-freewheel to match. A six speed system is the norm for this type of bike and the above parts will set you back in the region of £55. Labour in Bristol I would expect to be about £35-40 on top.
If the bike has a 1/8th chain on it already then you will need to change the chainset as well which will add another £15 or so.
